For quality incident reports, it is important to gather all relevant facts, and collect evidence. Photos can be attached to illustrate the incident with additional explanatory description. Finally, there will be an investigation phase on the incident over the course of the question-and-answer report.

The signature will make the report official, in case of legal or insurance problems or simply internally for administrative follow-up. Only the site manager and/or the QHSE manager can sign the form.
